Using your PAN Foundation grant is easy and flexible. You choose when and where to use the grant for eligible covered expenses.
How to check your grant balance
You can check the balance of a grant in two ways:
- Log in to your PAN portal account online, anytime 24/7.
- Call us at 1-866-316-7263, Monday through Friday, 9:00 a.m. to 5:30 p.m. ET to talk to our contact center representatives.
Copay grants
When you are approved for a PAN copay grant, you can use your funds right away. You choose the providers you use, which pharmacies you use, and which covered medications in PAN’s disease fund formulary you want to use it on that have been prescribed by your provider.
What’s covered
Check the list of medications under your disease fund’s formulary for what is covered by your grant. This list is unique to your disease fund. These formulary lists can be found on the PAN website disease fund pages, or in your portal account. The amount of copay assistance in your grant varies by disease fund.
How to use your grant
If you are the patient or caregiver, just give your PAN benefit card–that came with your welcome letter– and grant welcome letter to your provider or pharmacy. They can submit any claim or reimbursement for you, and the provider or pharmacy is paid directly by PAN. You can use your grant at a different pharmacy or with another provider at any time—this does not impact your grant status in any way. Just provide your PAN member ID and letter to the new provider or pharmacy.

How long grants last
Copay grants last for 12 months–called your eligibility period—after your application is approved. We provide grants on a rolling 12-month basis, in full compliance with all relevant laws, regulations, and government guidelines.
For your first grant, you can ask for reimbursement for approved out-of-pocket expenses paid before your grant was approved, if those expenses are within 90 days of when your grant started.
How to keep your copay grant active
Your grant lasts for 12 months but you, or your provider or pharmacy, must submit a claim or reimbursement every 120 days during your enrollment period. If you only need treatment once or twice a year and you get an alert from us indicating that you must use your grant soon, please contact us.
How to apply for more funds
If your current PAN grant balance is $0 but you need more money to pay for medication before your eligibility period ends, you can apply for additional funding if the fund is open. If your initial 12-month grant eligibility period has ended or is about to end within 30 days, and you need money for another 12 months, you may apply to renew your grant if the fund is open. Apply for additional funding and renewal grants online through your PAN portal account, or call us at 1-866-316-7263, Monday through Friday, 9:00 a.m. to 5:30 p.m. ET.
Health insurance premium grants
Health insurance premium grants cover the premium for the patient, or the patient’s portion of a family insurance plan. After you have been awarded a health insurance premium grant, we will pay your insurance company directly. If your insurance company is unable to accept direct payment from PAN, we will send you a reimbursement check.

Transportation grants
Activate your PAN debit card
Once you’re approved for a transportation grant, you will get a $500 prepaid Visa debit card in the mail within 15-20 business days. You must activate the card before you can use it the first time. Once you activate it, we’ll email you confirmation that your card has been activated. This process often takes one business day or less.
Making transportation purchases
To use your debit card, just swipe it using the credit card option. No PIN is required and there are no transaction fees. The funds are withdrawn from your grant account immediately, and you can check your balance in your PAN portal account. You do not need pre-approval each time you use the card.
Note that all card activity is actively monitored, and we can cancel your grant if the card is used for any unintended or inappropriate purposes.
Spending limit
The total daily spending limit is $350 with your debit card. There are also daily limits by category:
- Airfare: $300
- Car rental: $100
- Gas: $75 ($150 limit per week)
- Hotel: $300
- Parking: $50
- Rideshare: $100
- Taxi, bus, or train fare: $100
Your debit card will be declined if you go over a spending limit without permission. Remember: we can increase limits for individuals with special circumstances. If you would like to ask for a higher limit, call us at 1-866-316-7263 from Monday through Friday, 9:00 a.m. to 5:30 p.m. ET.
What’s covered
You can use your transportation grant to pay for transportation to and from activities that improve your overall health, such as grocery stores, medical appointments, pharmacies, or social support. Note: this grant only covers travel costs, not the purchase of goods or other services. Eligible expenses include:
- Airfare (tickets, fees)
- Car rental (car, insurance)
- Gas (only at the fuel pump, cannot be used to pre-pay inside)
- Lodging, such as hotels or temporary housing
- Non-emergency medical transportation (shuttle service)
- Parking (lot, meter fees)
- Public transportation fares (bus, metro, train)
- Rideshare (app payments, fees)
- Taxi (fare)
- Tolls (electronic payment)
This grant cannot be used for:
- Concierge services
- Food or groceries
- Gift cards
- Mileage reimbursement
- Non-transportation related goods or services
Be advised that all card activity is actively monitored. The purchase of unauthorized items, like gift cards, is considered fraud and will result in the immediate loss of the grant and card balance. We also have the right to cancel your grant if a transaction has not been made on your card in 120 days or if there is a change in your eligibility.
Receipts
You don’t have to submit receipts to PAN. However, we recommend saving your receipts in case of an audit.

Your grant lasts for 12 months but you must use your debit card to make a purchase for an eligible expense every 120 days during your enrollment period. Card help

- If you have not received your debit card 25 business days after you were approved
- If your debit card is declined for a covered expense, or a merchant you want to use doesn’t accept Visa
- If your card is lost or stolen
- To request a higher daily spending limit for one or more categories
Keeping your grant active
To keep your copay, transportation, or health insurance premium grant active, you must use the grant within 120 days of your enrollment date, and once every 120 days throughout your 12-month eligibility period. If you don’t, your grant will be canceled.
- For copay grants: you, or your provider or pharmacy, must submit a claim or reimbursement every 120 days
- For transportation grants: you must use your prepaid debit card to make a purchase for an eligible expense every 120 days
